To get a Berkeley Public Library card for a child through age 13, print out and complete the accompanying application form and bring it to any Berkeley Public Library location . You will need the Adobe Acrobat free software program to view and print the children's library card application form. The child must write his or her own name on the form. The form must be signed by the child's parent or legal guardian.
NOTE: The child must be present to receive his/her library card..

TEACHERS: you may print out this application and photocopy it for your students. If you have made an appointment for a class visit, please return the completed library card applications to the library site you will be visiting at least two working days before the visit.
