Getting a Library Card
To get a Berkeley Public Library card for a child under age 13:
- Children may get a library card when they are five years old.
- Print out and complete this Library Card Application Form
. This form is not interactive. Forms are also available at any Berkeley Public Library location. - The form must be signed by the child's parent or legal guardian.
- Bring the completed form to the library.
- The child must be present in the library to receive his/her library card.
Teachers: You may print this application form and photocopy it for your students. If you have made an appointment for a class visit, pleast return the completed library card applications to the library site you will be visiting at least five working days before the visit.
Remember:
- Bring your card with you to the Library every time. You will need it to check out books and other materials and to use the internet computers.
- You are responsible for all items checked out on your card.
- You may have up to 50 items checked out at any given time.
- Cards expire after two years. Renew the card at that time.
- If your address, phone number or email address changes, or if your card is lost or stolen, notify the Library immediately.
- There is a $1 replacement fee for lost, stolen, or damaged cards.
- Any California resident is entitled to a free Berkeley Public Library card. You do not need to live in Berkeley to get a Berkeley Public Library card.
